The Providence Community Health Centers, largest statewide network of community health centers, seeks a full-time 35 h! our/week Staff Nurse to provide clinical coverage for various Health Center Sites.
This position works collaboratively with providers and health center staff to provide comprehensive patient care in a primary care setting.
Responsibilities include, but are not limited, to the following :
Assessing immediate and long-term physical, mental and social health care needs for individuals and families.
Managing clinic flow by previewing schedules and patient charts, triaging and coordinating appointments, and maintaining medication logs.
Administering immunizations and medications and documenting all medical care.
Educating patients with acute/chronic health problems to promote and achieve health care goals through disease specific education materials, medication refills, nutritional recommendations and community support resources.
Act as liaison between health centers and patients to facilitate follow-up care.
QUALIFICATIONS:
Requirements include Associates or Bachelors Degree in Nursing, active RN License, 2 -4 years of Adult Medicine/Pediatrics experience in community health and/or hospital . Electronic Medical Records and Practice Management Experience preferred.
