JOB DESCRIPTION: Responsible for working as a liaison between Beacon and the provider communities and DCYF to control and/or decrease utilization by:!
Expediting access for members to services within the care continuum
Rapid response to provider/member and DCYF needs in community based settings and inpatient levels of care.
Decrease barriers to case managers being able to obtain signed consents for case management services.
Increase the amount of members enrolled in ICM via outreach and face to face engagement
Increase Beacons and Neighborhoods presence in the community
Increase DCYF case workers and key community providers understanding of our care continuum and ability to coordinate services through ongoing Beacon Case Management presentations.
PRIMARY RESPONSIBILITIES
Perform on-site case management at each of the DCYF regional offices on a schedule agreed upon by DCYF
Coordinate service delivery to ensure continuity and integration of services.
Assist members in accessing in and out of plan care by facilitating communication between coordinating agencies (CEDARR and DCYF).
! Identify individual limitations, deficits, and strengths and a! ggressively attempt to provide members with what they need in relation to medical, behavioral and social needs.
Help reduce the negative consequences to the individual when there is a lack of follow up and participation in treatment.
Access FlexCare at off site locations to expedite referrals to case management and educate providers with referral information for other services providers, levels of care, and overall care continuum.
Participate in daily rounds from alternative sites, when applicable.
Participate in member specific case conferences with DCYF staff, acute care facilities, and System of Care Network representatives.
Assist in analyzing barriers to timely movement of DCYF involved children from acute care settings and identify gaps in covered services.
Collaborate with emergency service teams (including local emergency rooms) to assist in expedited linkage to care for members when appropriate.
Obtain signed releases of information from! members/guardians via on site collaboration with providers.
Notify care coordinator of change in member demographic information to ensure timely notification to the plan.
Facilitate joint rounds with medical case management to assist in identification and out reach with medically compromised members.
Other duties as assigned

SCOPE: Case managers provide only case management service, they are not direct clinical service providers and do not conduct utilization review nor do they issue authorizations to providers.

EXPERIENCE/EDUCATION:
Case Managers are required to be educated in current principles, procedures and knowledge domains of case management based on nationally recognized standards of case management and must be a licensed behavioral health clinician. Re-verification to occur at a minimum of every 3 years.
A staff providing Case Management services will have at least one of the following:
Masters degree or high! er in a health related field and licensure as a behavioral health profe! ssional.
RN, licensure as a professional and 3 years behavioral health clinical practice
Licensed behavioral health clinician/nurse who has Certification as a Case Manager is preferred.

None

SUPERVISION RECEIVED:
Weekly by Team Lead

WORKING CONDITIONS:
Licensure required with proof on date of hire. Re-verification of licensure to occur at a minimum of every 3 years.
General office environment. May require time commitments beyond 40 hours per week at times. Travel required. Required background checks.
